Physicals/Drug Test will be May 11th from 1:00 to 4:30 pm. Cost will be $10 for the physical (no charge for the drug test).
 
*Every student who will be participating in extra-curricular activities must have both a physical and drug test on file.
 
***Parents, you must fill out and sign the OSSAA Physical examiniation and Parental consent form and the Drug Test Information form. 
 
***Completed forms must be brought back on May 11th in order for your student to be able to take a physical.
